Does my child need a Sports Physical?

If they are in middle and/or high playing school sports.

They must have a sports physical every year before practices, tryouts, or games.

Middle School Students

Annual sports physical required

FHSAA EL2 form must be completed and signed by a licensed provider

No ECG (heart test) required


High School Students

Annual sports physical required

FHSAA EL2 form completed by a licensed provider

NEW: Starting the 2026–2027 school year, first‑time high school athletes must have a heart screening (ECG/EKG)


Sports physicals help ensure student athletes are healthy and safe to participate.

What is an EKG?

❤️ What Is an ECG (EKG)?

An ECG (also called an EKG) is a quick, painless test that checks how the heart is beating.

Small sticky sensors are placed on your child's chest, arms, and legs. The test takes only a few minutes.

No needles, no pain, no shocks


The ECG helps detect hidden heart conditions that may not show symptoms but could be risky during intense physical activity.


The ECG is in addition to, not a replacement for, the sports physical. The ECG will be good for 2 years; a sports physical must be done yearly.


❗ What If the ECG Shows a Concern?

Most ECG results are normal

If something unusual is found:

It does NOT automatically mean your child cannot play sports

Your child may need further evaluation

Written medical clearance will be required before participation





Why This Requirement Matters

The ECG requirement was created to:

Detect rare but serious heart conditions early.

Prevent sudden cardiac events during sports.

Keep student athletes healthy and safe!
